I don't have any experience managing a network of 50 million end devices to be able to answer the 1st question exactly, but apparently that is how Comcast designed their management and provisioning systems and also apparently the pain of IPv6 migration is less than the pain of changing those systems and/or the pain of operating them with out "global" visibility of the network.
