This problem was around 5 years ago when I worked for Actrix, though nowhere near as bad as i have seen it recently. Recently my previous employer switched from using telstras dial offload back to IPnet. Since then I would hazzard a guess at over 50% of the customers where ending up 'stuck online' due to our AAA not receiving the stops. Infact, there were periods of hours which ALL stops, stopped. Sure plays hell when you offer time based accounts! Fortunately though, this is now someone elses problem :D
