http://webpages.netlink.co.nz/~alastair/toc-2-1.html It's not exactly the most elegant code (using MS Word's "Save As HTML" - urg) and my method of printing the diagram and table then scanning them into a file is surely the beginning of a great leap backwards, but it's enough that people can read the document.
